Named Accountable GP

From 1st April 2015 we are required under our NHS contract to allocate a named accountable GP to all patients who will be responsible for your overall care. This has been done for all existing patients, and all newly registered patient will be given a named accountable GP on registration.

 

 In order to facilitate this we have allocated the GP’s to patient on a surname basis as follows:

Patients with the surname beginning    A – B  : Dr Victoria Bastain

Patients with the surname beginning    C – E  : Dr Jon Sutton

Patients with the surname beginning    F – H  : Dr Helen Coundon

Patients with the surname beginning    I – L  : Dr N Lawson

Patients with the surname beginning    M – Q : Dr Reshmi Salam

Patients with the surname beginning    R – S : Dr Andy Roberts

Patients with the surname beginning    T – Z : Dr Hannah Tomlin

 

If you are aged over 75 and registered before 01 April 2015 you will already have been allocated a named accountable GP by letter – this named GP will remain the same and will not change under the surname arrangements above.

 

Please be aware that you may still see any GP of your choice at the surgery and do not have to see your named accountable GP, nor does this mean that you will only be offered appointments with your named GP.

 

As of the 31st March 2016 all of our patients have been allocated and informed of their named GP.
